What identification documents do I need to register?

1.      Proof of identity (passport, driving licence, birth certificate etc.)

2.      Proof of address (utility bill, official letter confirming address)

3.      Record of vaccinations for children under 5 years old

Speak to us if you do not have all documents required.

 

How do I register?

You can either register via the service below or collect a registration pack from the practice (you can download the forms below as well).

To apply for a registration with the practice online, please head to: https://gp-registration.nhs.uk/K83013

We'll ask questions about:

  • your name and address
  • the name and address of the GP practice you're currently registered with, if you have one
  • your NHS number, if you know it
  • the name and address of an emergency contact
  • the name and address of your carer, if you have one
  • your medical history
  • any allergies you may have
  • any medication you may be taking
  • your general health
  • a way to contact you such as a phone number or email address

You'll need to complete your online application in one session, so please have these details to hand before you start.

 

Once registered, you will be allocated a named GP but can see any doctor in the practice. If you wish to know who your named GP is, please check with reception after you have been registered.

All new patients can have a registration health check. Please ask the receptionist to make an appointment for you after registration if you would like one. It will be undertaken by a member of the Practice Nurse Team and will include a check of your blood pressure, weight, height and urine. You will be asked to provide relevant information about your health background.

Anyone who is living permanently within our Practice Boundary

Our practice covers:

  •       Cranford
  •       Thorpe Malsor
  •       Warkton
  •       Weekley 

 

Other information:

Please note we cannot offer you an appointment with a doctor or nurse until you are registered.

If you need additional help with any aspect of the registration process, please do not hesitate to ask one of our reception team.

Medical cards are no longer issued by the Health Authority.

If you wish to opt out of Summary Care Record (SCR), please request an opt-out form from reception. You can find more information about SCR on our website: http://www.eskdaillmedical.co.uk/medicalrecords.html

We will only register patients who are entitled to NHS care and who are currently residents within our practice area (please see Practice Boundary details in General Information). Further details may be obtained from our reception team.

We will not register children under 16 without a parent/legal guardian.

 

Military veterans

Do let us know if you are a military veteran. You may benefit from priority treatment.
